Notes:

1.

Students, staff, and faculty are eligible to use the Core XM License for academic use. (Note: College of Nursing students must use REDCap.)

2.

Students who need to join a specific organization's division, should refer to the “Join a Qualtrics division” section below.

3.

Faculty and staff are not eligible to use any of the Qualtrics Core XM Licenses for personal use. 

4.

Faculty and staff may use the Qualtrics Core XM License for university related purposes. If they need to collaborate with users within or outside of their organization, they need to check the Department & College Licenses web page to find out whether their OrgID already has a license.

If an organization does not have a single or parent ORGID license, they should refer to the “Access the product” section below for employees.


Product information

Qualtrics is a user-friendly online survey tool that allows you to build surveys, distribute them to participants, monitor responses, and analyze the results.

Features include:

  • Intuitive survey design interface with question libraries and automatic choices
  • Comprehensive lists of question types including pick, drill down, rank order, etc.
  • Survey development with collaboration options
  • Data analysis

License information

The University purchased a campus-wide site license for the Qualtrics Core XM survey platform for academic use, which is managed by the Office of Software Licensing (OSL).

University of Utah Health has also procured a Qualtrics license agreement. If you intend to survey patients, please complete the Patient Experience Qualtrics Request form in ServiceNow. If you have questions, contact the Patient Experience team at epe@hsc.utah.edu. If protected health information (PHI) will be collected or accessed as part of your use of this license, contact the University of Utah Health Privacy Office at baa@utah.edu

Acceptable use

The Qualtrics software products provided under the campus-wide license agreement may be used by students, staff, and faculty of the University of Utah for any purposes related to teaching, learning, research, and administrative activities.

Access the product

Students

Students are eligible to use the Core XM License for academic purposes. (Note: College of Nursing students must use REDCap.) Sign into https://utah.qualtrics.com/ using single sign on  and your university credentials (uNID@utah.edu and CIS password).

If students need to join an organization’s division, they need to review the list of Department & College Licenses and send a request for access to the administrator listed. Include your first and last name, uNID, and your professor's name.

After you receive the upgrade code from the administrator, follow the instructions under the “Upgrade your account to a division” section below.

Employees

Employees are eligible to use the free Qualtrics Core XM license. Sign into https://utah.qualtrics.com/ using single sign on and your university credentials (uNID@utah.edu and CIS password).

If you need to collaborate with users within or outside of your organization, you need to review the list of Department & College Licenses and send a request for access to the administrator listed. Include your first and last name, your uNID, and reason you need to join the division.

If your organization is not on the list, then you need to submit a registration form for a division to be created on the campus-wide Qualtrics website.

The following division types are available:

  • SingleOrgID license: Department/administrative unit license for a single specific OrgID
  • Parent + Roll Up OrgID license: College/organization license for a parent OrgID and any OrgIDs that roll up into the parent OrgID

After you complete the form, OSL staff will review your response and contact you with additional information.

Join a Qualtrics division

If you need to collaborate with others within your organization and your organization appears on the list of Department & College Licenses, please send a request to the administrator associated with the organization and request to join the division.

Include the following information:

  • First and last name
  • uNID
  • Reason to join

If your request is approved, the division administrator will share their upgrade code to join the division.

Upgrade your account to a division

  1. Sign in to Qualtrics using single sign on and your university credentials (uNID@utah.edu and CIS password).
  2. In the upper right corner of the browser window, select your profile icon.

  3. Select “Account Settings”.

  4. On the User Settings tab, in the notification box at the top of the window, select Switch to older version.

  5. On the menu on the left side, select “Upgrade Account”.

  6. In the field for “Upgrade Code”, enter the code you received from the Office of Software Licensing or your division administrator and select the “Upgrade Account” button. Wait a few minutes for the change to take place.
  7. Select your profile icon in the upper right corner of the browser window, and select “Refresh Account".

Collaborate with users outside of your division

If you have joined a division, the division administrator of your organization can create collaboration groups and can add collaborators provided they have a Qualtrics license or they are included in another division.

Leaving the U?

If you need to transfer your survey(s) to someone in your division, please contact the Campus IT Help Desk (801-581-4000 or helpdesk@utah.edu) and request that they createan incident for the Office of Software Licensing. Upon receiving the incident, OSL staff will respond to your inquiry.

Support

If you gained access to the software through your department or organization, please contact an  administrator for your department, college, or organization from the list of Department & College Licenses.
