Students, Staff, and Faculty are eligible to purchase and use the FREE Qualtrics Core XM - Trial License. NOTE: Users are limited to two surveys with up to 10 responses each using the trial license.
Other than the FREE Qualtrics Core XM – Trial License, students are not eligible to purchase Qualtrics Core XM licenses. Students may gain full access to the Qualtrics survey software features via their department or college of study. For details, see question 5 in the FAQ below.
Faculty and Staff are not eligible to purchase any of the Qualtrics Core XM licenses for personal use. Faculty and Staff may gain full access to the Qualtrics survey software for university related purposes. For details, see question 5 in the FAQ below.
Faculty and Staff may purchase the Qualtrics Core XM – Single User License provided they only use the software for university related purposes.
Prior to purchasing a Core XM - Single User License, the Purchaser must first check the department & college licenses web page to see if their ORGID already has a license.
Faculty and Staff may purchase the Qualtrics Core XM – Single ORGID License and/or the Core XM – Parent + Roll Up ORGID’s License. The Purchaser of these licenses must designate a Division Administrator who will administer and manager the license for the specified ORGID.

Qualtrics is a provider of online survey software. The online survey tool is user friendly and allows you to build surveys, distribute to participants, monitor responses and analyze the results. Features include:
- Easy survey design with question libraries and automatic choices
- Comprehensive lists of question types including pick, drill down, rank order, etc.
- Survey development with collaboration options
- Data analysis
Yes. The University purchased a campus-wide site license for the Qualtrics Core XM survey platform for academic use, which is managed by the Office of Software Licensing (OSL).
This license is not covered with a BAA and is not HIPAA compliant.
This license is NOT eligible for patient surveying. If you need a University of Utah Health license to survey patients, please see Patient Experience Qualtrics Request in the Service Now Catalog. Direct any questions to the Patient Experience team at firstname.lastname@example.org.
The Qualtrics software products provided under the campus-wide license agreement may be used by students, staff, and faculty of the University of Utah for any purposes related to the University of Utah teaching, learning, research, and administrative activities.

Students have access via their department or college of study. Review the list of department & college licenses and send a request for access to the administrator listed. Include your first and last name, uNID, and your professor's name.
Faculty & Staff
Although the license is free, you need to purchase a license on our web store. There are three options available for purchase:
- Individual license for Faculty and Staff
- Department/administrative unit license for a single specific OrgID
- College/organization license for a parent OrgID and any OrgIDs that roll up into the parent OrgID
Before purchasing a license, review the list of department & college licenses and if your department, college, or organization is on the list, send a request for access to the administrator listed. Include your first and last name and your uNID.
If your department, college, or organization is not on the list, then individual licenses can be purchased on the OSL web store.
If you sign in without purchasing a license, you will not have access to the Core XM survey platform. To get access to the survey platform, you need to purchase a license at the OSL store.
Go up to your profile icon in the upper right corner of the browser window, click and hold.
Select the option “Account Settings”.
On the menu on the left side, select “Upgrade Account”.
In the field for “Upgrade Code”, type in the code you received from the Office of Software Licensing or from your division administrator and press the “Upgrade Account” button. Wait a few minutes for the change to take place.
Click on your profile icon in the upper right corner of the browser window and select the option “Refresh Account".
If you have purchased an individual license and your department, college, or organization appears on the list of department & college licenses, please send a request to the Office of Software Licensing and include the following information:
- First & last name
- OSL order number
- OrgID you want to be added to
A member of the Office of Software Licensing team will verify the validity of your request and will reply with the status of your request.
If you have purchased a license, you will be notified before the license expires that it is time to renew the subscription.
You will have the choice to renew your subscription. If you choose not to renew your license, your access to your account on the license will be suspended.
If your access is suspended, you will not have access to your surveys and data. If you obtained access to the Qualtrics software through your department or college, contact your division administrator for assistance. If you purchased a Single User License, please contact the Office of Software Licensing for assistance.
If you gained access to the software through your department, college, or organization, please contact one of the administrators for your department, college, or organization from the list of department & college licenses.
If you purchased an Individual single user license from the OSL web store, please send an email to email@example.com and someone from the Office of Software Licensing will contact you.
